We provide medically-tailored groceries and prepared meals, dietary supplements, and comprehensive nutrition services throughout all of Sonoma County.

Jump to a program:

  • HIV Nutrition Program
    Medically tailored weekly groceries for individuals with a positive HIV diagnosis, provided for as long as needed.
  • Welcome Home Program
    Three months of medically tailored groceries and/or meals following an in-patient hospitalization.
  • Infectious Disease Program
    Medically tailored groceries delivered weekly, with duration determined on a case-by-case basis.
  • Oncology Nutrition Program
    Three months of medically tailored groceries or meals for the client and their household. Service duration may be extended based on the course of treatment.
  • Full Belly Program
    Medically tailored groceries and meals for expecting clients and their household, continuing until the baby reaches 3 months of age.
  • Community Supports Program
    Twelve weeks of medically tailored groceries and meals for patients with Medi-Cal, provided they have medical justification for the services.
  • Bags of Love
    Ready-to-eat meals for organizations to distribute to their unhoused patients.

Welcome Home Program

 
Who’s EligiblePatients being discharged from an in-patient hospital or mental health hospital stay within two weeks of their discharge date, plus household members. Must score 6 or higher on DETERMINE Nutrition Checklist (screening is on back of the referral form).
Services ProvidedFree weekly groceries, including fresh produce, high-quality protein, healthy grains, vitamins, and supplements.
Medically-tailored prepared meals, if needed.
Home delivery, if needed.
Individual nutrition counseling.

Who’s EligiblePatients undergoing cancer treatment, plus household members.

Individual nutrition counseling.
Nutrition supplements, such as Ensure and protein powder, if needed.

Full Belly Program

 
Who’s EligiblePatients who are pregnant and at risk for malnutrition, plus household members.
Services ProvidedFree weekly groceries, including fresh produce, high-quality protein, healthy grains, vitamins, and supplements.
Medically-tailored prepared meals, if needed.
Home delivery, if needed.
Individual nutrition counseling.

Infectious Disease Program

 
Who’s EligiblePatients with a positive COVID-19 test, Tuberculosis, or Monkeypox diagnosis or patients who are debilitated by Long COVID, plus household members.
Services ProvidedFree weekly groceries, including fresh produce, high-quality protein, healthy grains, vitamins, and supplements.
Medically-tailored prepared meals, if needed.
Home delivery, if needed.
Individual nutrition counseling.
Program DetailsVaries by need. Food For Thought’s client services team will be in touch with patients on a weekly basis to determine program length.

Community Supports (ILOS) Program

 
Who’s EligiblePatients with Medi-Cal and medical justification for medically tailored groceries or meals. A medical justification is required. Examples include but are not limited to: 

A client with poorly controlled diabetes who can benefit from diabetes friendly groceries to help stabilize blood sugars and get used to eating foods that will help her health.

A client with issues that make it hard for them to shop, e.g. physical disabilities, while also coping with GI issues that require specific foods or diets.   
Services ProvidedFree weekly groceries, including fresh produce, high-quality protein, healthy grains, vitamins, and supplements.
Medically-tailored prepared meals, if needed.
Home delivery, if needed.
Individual nutrition counseling.
Program Details12 weeks; possibility of extension depending on medical condition(s)

Bags of Love

 
Who’s EligibleNutrition for your patients experiencing homelessness and serious medical conditions.
Services ProvidedOur Bags of Love program provides ready-to-eat food to patients who are experiencing homelessness and serious mental and/or physical health conditions. 
To streamline the distribution process, we partner with local health centers who work directly with people without housing. These organizations distribute the bags of food onsite at their clinics and offices. 
Each bag of food has the caloric value of 11 meals and include the following items:
Crackers
Peanut butter
Canned fruit
Canned chili
Cup of soup
Canned chicken or tuna
Utensils
