Any Sonoma County resident diagnosed with HIV is eligible to receive Food For Thought’s comprehensive nutrition services - free of charge.
The intake process is simple: Come to the food bank, fill out an application, and show us proof of residency and letter of diagnosis from your doctor. We’ll spend a few minutes telling you about our services and procedures, and you can go home with groceries the same day!
If you are not able to travel to our facility in Forestville, please give us a call and we will make other arrangements for you.

Welcome Home Nutrition for Healing
Any Sonoma County resident who is released from the hospital or care facility and is at risk of malnutrition is eligible for three months of our comprehensive nutrition services - free of charge.
This program is designed to help patients heal and avoid returning to the hospital. More information about this program is available in the Welcome Home brochure.
Hospital staff or medical providers will assess patients and must refer them within one week of discharge. To refer a patient, providers should download and fill out the patient referral form, then fax it to 833-251-6560. More information is available in our referring provider flier.
